As the leader of Digital Transformation, my primary objective is to guide service improvements specific to digital access pathways for our patients. To achieve this, I must identify opportunities to make positive transformation, and provide the inspiration, resource and tools to deliver changes in a collaborative way.In line with this, I must plan, coordinate and directly support delivery of transformation activity in general practice within our PCN – with a view to improving patient access, staff and patient satisfaction, and the efficiency and sustainability of our general practice services.My role requires me to work closely with staff working at practice and PCN level to understand current service models, identify challenges, agree priorities for improvement and then support these to be delivered. To deliver successful digital transformation I must use my experience in business / service improvement and expertise in general practice systems and processes, including data driven analysis to measure the impact and improvement of our changes. As with any change, I must also manage the transformation of services for both patients and staff, and other key stakeholders. Successful change management requires clear communication and engagement with key stakeholders to bring people together, and gain support and buy in with the transformation of our current service processes. While working in this temporary contract as a secondment to my permanent role as Patient Services Manager, I must also manage my time effectively and prioritise my responsibilities to ensure that the operational services within our PCN remain safe and effective day-to-day.

As an RTT Validator, my primary role is to accurately analyse and evaluate pathways to deadlines whilst having an empathetic approach that places patients care above all else.My responsibilities cover areas such as but not limited to:Delivering detailed validation of referral to treatment pathways and reviewing patient case notesIdentifying trends or constraints within specialities and highlighting areas of concern for review and possible redesign to the relevant Operational TeamsTo ensure data is validated and a robust audit trail kept throughout the journey and that data is entered onto local 18 week pathway databasesProvide expertise in proactive management of pathways from referral to discharge to Admin, Operational and Clinical Teams ensuring pathways are managed smoothly to deliver a high standard of patient satisfactionDeliver training to staff new to post as well as Admin, Operational and Clinical Teams where requiredTo understand 18 weeks referral to treatment rules and use them to validate all elective patient pathwaysUndertake daily validation of the RTT PTL and take action or escalation of incomplete pathways to ensure patients receive timely treatmentReview case notes and documents to establish an accurate record on both complete and incomplete pathwaysCommunicate effectively with Operational Managers to highlight any bottlenecks, ensure staff are aware of key dates in the patient pathway and potential delays or breaches of key milestonesProduce reports for circulation to Operational and Clinical Teams identifying actions required on pathways nearing breachTo use multiple clinical information systems and databases to understand and coordinate patients' pathwaysMonitor Out-patient clinic outcome and Treatment Status for incorrect data input electronically in order to i) Escalate to relevant Operational Teams ii) Contact user to arrange training to confirm correct process to be followed iii) Record accurate outcome and status
