Planned, developed, and implemented a new hospital-based affinity program for adults 50+ using extensive knowledge of aging services (current membership 10,500+).Designed and implemented ReConnect Leadership Project, an innovative model of unpaid civic service within health care institutions which uses highly skilled, highly trained volunteers to enhance communication among physicians, staff, patients, and families especially around discharge information, and to provide transition intervention coaching to reduce hospital readmissions.Secured more than $800,000 in funding from foundations and government entities to sustain Health Passport programs in FY11.Designed programs to provide one-on-one counseling to 8500 individuals in FY 2011 in the areas of medication management, prescription assistance, Medicare, Medicare Part D, Long-term Care Medicaid, Medicare Savings Programs, and other public benefits.
