Developed program plans and evaluations to implement, scale-up and sustain interventions. Broadly, my role involved: Conducting implementation research and evaluation projects that were outcomes and impact oriented. Tailoring implementation protocols for evidence-based interventions that were adapted to local contexts and aligned with user resources, skills, values and needs. Identifying implementation and evaluation frameworks that met project objectives. Supporting OHTN and partners in evaluating, sustaining, and scaling effective interventions. Collecting and analyzing quantitative and qualitative information to monitor program progress using CQI methods. Delivering evaluation learning and capacity building initiatives for OHTN clients.

iCAN ACP is a research project that aims to narrow the gap between the care that frail elderly Canadians want and the care that they receive by testing ways to improve advance care planning (ACP), a process for reflecting on and communicating wishes for future health care.
