Quality Improvement Practice Leader
CurrentThe Quality Improvement Practice Leader focuses on improving the health of individuals, families, and children who receive health care under Child Health Plan Plus (CHP+) and Health First Colorado (Colorado's Medicaid Program). Our focus is driving improvements in quality, member experience, outcomes, and cost through practice facilitation. • Supports providers in implementing process improvements to assist them in meeting program outcomes, especially for value-based payment models.• Supports providers in using practice-level population data to improve access to care, create registries, identify opportunities for process improvements, and evaluate outcomes. • Assists practices in the identification of appropriate target populations, such as high-risk members, and develops and implements systematic strategies to improve care and outcomes.• Assists practices in developing programs to reduce inappropriate emergency department utilization, hospital admissions, readmissions, or other identified program goals using data and process improvement techniques.• Assists practices in adopting process improvement capabilities creating workflows to optimize care delivery, training in best practices improvements, and evaluating outcomes.
