CurrentProvide social evaluation, assessment, counseling, and crisis intervention services to consumers with severe and persistent mental illness and substance use disorders who are in need of intensive, home or community based servicesProvide evidence based, clinically effective individual therapy while meeting all regulatory requirements to ensure reimbursementProvide evidence based, clinically effective group therapy focusing on Stages of Change, and social skills management/developmentConsultation with various community stakeholder agency staff, law enforcement, family members or support network as deemed necessary or clinically appropriateInitiate, and revise as necessary, Person Centered Plans and Crisis PlansSubmit service authorizations or reauthorizations to MCOs with enough time to avoid service interruptionEnter program service goals for new consumers and when Person Centered Plans are updated to maintain current goal progress and other applicable information
