Assist and give temporary relief to the full time caregivers of kids with special needs (mostly autistic), by interacting/playing, helping with basic life skills (hygiene, feeding, changing clothing, etc.), taking kids on outings, tutoring in math, reading skills, and spelling.

Conducted assessments and ongoing individual counseling with young adults, where I documented each session through case notes, and made sure each case file was updated. I also helped administer drug tests at high schools, and participated in conducting drug/alcohol history activities in the JADE (juvenile alcohol drug education) program that high school students and a parent were required to attend when the the students would get caught with drugs, alcohol, or drug paraphernalia on campus.

A court replacement facility environment (group home setting). I worked closely with 13-18 yr. olds by supervising, and monitoring them at all times. I counseled them through their daily struggles, transported them to court appearances, doctor’s appts., therapy appts., school, etc. I monitored family visits and documented them, where such documents were used in court. I also took care of high crisis situations, where it was required to submit special incident reports and communicated with law enforcement, when such incidences would occur.

Counseled 19-24 yr. olds who were preparing to live on their own. I took care of the bookkeeping for the residents rent, posted current job openings for residents, updated residents case files, dealt with high crisis situations where it was required to submit special incident reports.

Conducted and documented psychosocial assessments on patients in home settings, facilities, and environments that had medical personnel present but, were less institutionalized. I also participated in weekly meetings where the VITAS’ doctor, registered nurses, caregivers, chaplain, and my supervisor, licensed clinical social worker were present. During these weekly meetings VITAS employees would discuss each patient they were treating, and what their plan was going to be for ongoing care.

Assisted in monitoring youth and making sure that they stayed on schedule for school and other responsibilites. Performed timely bedchecks, and provided overnight security.
