    I am assisting Case Managers who have participants that are considered high utilizers of the Emergency Department and we working together to decrease their Emergency Room visits. I follow up with participates who are on the Mercy Care Gaps of Care list to complete their annual preventative care by setting them up with our Primary Care Physician (PCP), or making their appointments with their outside PCP. I contact participants who missed their appointments with Gateway’s clinic PCP and rescheduling them.
