*Obtains and reviews patient's medical history and conducts physical to determine a plan of care for medically-managed weight loss*Orders and reviews laboratory studies such as lipid panels thyroid level, comprehensive metabolic panel and complete blood count*Orders and reviews diagnostic procedures such as EKG*Assume responsibility for prescribing FDA approved appetite suppressants according to state and federal guide lines*Review patient's progress and manages patient care accordingly*Maintain all required documentation for patient care and medication management*Conduct patient consultations and provides preventive medicine counseling*Document patient progress notes in the patient chart and/or EMR*Perform other clinical duties as required to support the team

*Provide patient-centered care management to patients identified from high-risk registries*Coordinate with other members of the care team at multiple physician practices within a large hospital system*Develops individual plan of care, provides care coordination, develops prevention plans, and ongoing monitoring*Provide proactive care and education outside of the routine office visit via telephone*Builds a trusting, professional relationship with the patient, family, and/or caregivers
