Our emphasis is on high acuity post-acute patients in order to achieve best outcomes including readmission reduction, medication reduction with a strong emphasis on transitions to and from the skilled unit. The vast majority of our physicians have transitioned from Hospitalist to the Skilled Rehab arena, and bring their valuable skills to the post-acute unit.Due to an attractive business model which emphasizes provider autonomy, involvement in management and low overhead, we have grown from 2 to 15 providers and from 2 to 19 facilities in the past 5 years. We have increased our presence in the SNF/LTC market through word of mouth, attracting those facilities who are looking for daily presence of an NP and 3 times weekly presence of a physician with weekend rounding and stability of providers. We believe in maintaining relationships and retaining physicians and facilities based on our emphasis on clinical excellence and mutual respect and avoid any non-compete restrictions with both providers and facilities in order to avoid any interference with facility and provider choice.
