As a member of a primary care and behavioral health team, I provide services to patients in the primary care clinics. Services include working with the Primary Care Provider (PCP) and Behaviorist to assist patients with access to community resources (e.g. food, shelter, clothing, and benefits), support delivery of services that are culturally and linguistically competent, assist in completion of routine screening and outcome measures, manage Behaviorist patient flow, and support Behaviorist services.

Assist individuals in assessing their skills in retail, landscaping and janitorial work. I head the janitorial program. I provide clients with job development, help with the search process and provide them with on and off the job training at their places of work. I am an advocate for individuals with disabilities. I make sure that both my clients needs and their employer's needs are met. I manage a caseload of 11 clients and keep in touch with their Department of Rehabilitation and Golden Gate Regional counselors to ensure that my clients needs are met and that they accomplish their goals.

Responsible for planning/conducting activities for Assisted Living Residents. Motivated residents to participate in arts, crafts, games, parties, entertainment events, and movie night. Kept a record of attendance and participation for each resident.
