• Acute care learning experiences in internal medicine, critical care, emergency medicine, family medicine, and specialty services (i.e. infectious diseases and cardiology) at a teaching facility involving attending physicians, medical residents, medical students, and pharmacy students • Longitudinal ambulatory care clinic experiences (i.e. anticoagulation, diabetes, congestive heart failure, pain management and addiction) with collaborative practice agreements and/or direct recommendations made to attending physicians • Longitudinal antibiotic stewardship to monitor patients started on vancomycin, aminoglycosides, restricted antibiotics, Coumadin, and digoxin for antibiotic optimization or de-escalation and to adjust dosages and draw drug levels of vancomycin, aminoglycosides, and coumadin based on pharmacy protocol and policy• Completed quality improvement projects in medication use evaluation (fentanyl patch), development of annual antibiogram, ward inspections, and medication error reviews• Participated on P&T meetings to discuss formulary changes and requests• Precepted University of the Pacific (UOP) pharmacy students on APPE clerkship through organized health fairs or on their core learning experiences on internal medicine, family medicine, cardiology, infectious diseases, or critical care• Created presentations and inservices to multidisciplinary audience (i.e. nurses, attending physicians, medical residents, medical interns, medical students, pharmacy students, and clinical pharmacists)• Assisted in inpatient operational duties 8 hours a week to input and verify orders to ensure timely delivery of medications and medication safety
