Senior Care Manager• Serves as the primary point of member contact, manage case load, including risk stratification of members, monitoring reassessment needs and facilitating transitions of care settings.• Develop, implements and monitor the person centered service plan• Review UAS and assesses member needs, manage care and services, and ensures effective communication among members, caregivers, providers and community supports• Lead the interdisciplinary team, and facilitate the activities and communication within an interdisciplinary team of providers, vendors, and for member facility discharge• Ensure continuity of care for newly enrolled members, identify and prioritizes the member’s needs and preferences• Develop quantifiable goals and desired outcomes, and promote the member’s ability to self-manage to the greatest extent possible.• Assist members in obtaining reasonable accommodations when appropriate

UAS Assessor• Conduct in-home assessments and enrollments for Maximus to evaluate the consumers Medicaid eligibility and ability to perform activities of daily living and the degree to which the client requires assistance for each function and task. • Enter the completed assessment into the UAS –NY New York State Department of Health

CHHA Home Care RN Duties • Supervise nursing assistants and care of assigned residents• Administer medications and treatments according to physician orders and scope of practice• Communicate with physician, residents, and family regarding resident status and care• Complete appropriate documentation and charting including incident reports, orders, and care plans enter completed assessment in to OASIS/McKessonUAS-NY MLTC RN Duties• Conduct in-home assessments and enrollments for MLTC to evaluate the client’s ability to perform activities of daily living and the degree to which the client requires assistance for each function and task. • Enter the completed assessment into the UAS –NY New York State Department of Health • Develop a plan of care for the type and amount of home care services to be provided to the client after reviewing all required case documentation• Assist in determining the level of home care services or other services of care that the client needs based on the medical diagnosis and degree of functional impairment.
