As intern, I greet, facilitate and review the initial paperwork and self-assessments patient completes. These documents include: The Outpatient Summary List, Substance Abuse History and Substance Abuse Assessment. Secondly, I was responsible for orienting the new patients to the program and the facility. Over the course of the intake process I assessed the patient with completing The Psychosocial Assessment or the Psychosocial Update. With the patient, we develop goals and complete the treatment plan and treatment plan updates. During morning check in group I was responsible for screening and breathalyzing the clients in a randomized fashion. Lastly, I recorded those results in their chart and following up with the patient about positive screens.
