CurrentOverviewPerforms therapeutic services including assessment, counseling and case management of assigned behavioral health patientsResponsibilitiesProvides individual and group counselingServes as primary contact/clinical liaison to patients and patient’s families to ensure delivery of effective and appropriate treatment in accordance with established plan of careReviews charts of assigned patients to determine appropriate referral services based on recommended treatment planFacilitates, schedules and/or participates in clinical team meetings to monitor patient progress against goals; updates progress notes in Claim Track and patient chart of all patient contacts within established timeframeCollaborates with patient, family members and other involved parties to assess barriers to attainment of goals and/or resourcesMonitors eligibility status with AHCCCS; initiates action to re-instate eligibility to ensure continuity of treatmentResponds to emergency and/or crisis situations; contacts appropriate individuals and/or agencies to ensure appropriate intervention and actions to optimize patient safety; refers difficult and/or high risk cases to Team Lead as neededWorks closely and cooperatively with Child Protective Services, Tribal Social Service and courts acting as point of contact for communication/coordination of care between providersManages and oversees care coordination of assigned case load to ensure delivery of all covered services identified for continuity of care across all levels of care and involved providersDevelops and implements transition, discharge and aftercare plans of assigned patientsPerforms Behavioral Health assessment of patient, develops treatment/service plan to meet determined care needs; revises care plan as appropriateInvestigates and follows up on patient complaints to ensure timely response and/or appropriate resolution to concerns/issuesIdentifies appropriate community resources and performs follow up as appropriate
