Completed assessments under the Care Act (UK Adult Legislation) to identify care needs of individuals aged 18+ and arranging services to support including care packages. Provided services to people who required support with dementia, learning disabilities and mental health.Completed assessments and carried caseloads in hospitals, care homes and the community.Worked as part of the hospital team, attending meetings on a daily basis for patient updates.Worked as a part of a Duty Team as first responders to referrals, determining level of risk to the person and their families and completing a screening tool to determine whether it requires urgent assessment.Supported people with debt advice and referred them to services who can support with benefit applications and set up repayment plans.Gathered information for safeguarding concerns. Worked with local providers and the Commissioning Team to ensure the person who is at the center of the safeguarding was protected.Collaborated with local providers including financial advisers and health professionals.Ensured caretakers were protected and referred them for Carers Assessments and services.
