 Implementing growth of Transitions of Care (TOC), Special Needs Population (SNP) Model of Care (MOC), Chronic Kidney Disease (CKD), Complex Case Management, Organ transplantations and Inpatient departments.  Direct manager of up to 24 employees per Optum/United Health Group closely following clinical languages of leadership guidelines.  Facilitation of MOC meetings bi-weekly per Centers for Medicare & Medicaid Services (CMS) guidelines and National Committee for Quality Assurance (NCQA) standards.  Support for inpatient services, Diagnostic Related Groups (DRG), Detailed Explanation of Non-Coverage (DENC), Notice of Medicare Non-Coverage (NOMNC), and Post Stabilization.

 Providing expert level case management services to the most chronically ill populations.  Drive organ transplant case management services from evaluation through post-transplant phases.  Delegating assignments of low to high-risk Members throughout case management department.  Enhance quality of life through MOC and facilitation of MOC Interdisciplinary Teams (IDT) meetings.  Support and training as a Team-Lead throughout inpatient and outpatient departments.

 Transitioning case load from acute inpatient to outpatient levels of care.  Assess, diagnose, plan, implement, and evaluate care  Direct communications with acute ill populations, providing resources to ensure stability.  Coordination of care providing best practices throughout populations

• Coordinate mental/physical health and medication support services to members enrolled in multiple programs (PATH, Wellness DMH, LAHSA) in the homeless, substance abuse, psychiatric demographic.• Administration of medications, monitoring for side effects/ adverse reactions and response, as prescribed by Psychiatrist and Physicians.• Implementation of procedures and management of medication storage rooms, maintenance and proper storage and disposal of medications.• Documentation of all activities within requirements of Department of Mental Health.• Reporting medical changes and networking daily with treatment teams, including case managers, client advocates, psychiatrists, social workers, psychologists, pharmacists and community housing.

• Provide case management and medication support services to consumers enrolled in the program.• Daily assessment of mental and physical health including substance abuse and proper referrals, appointment setting and educating.• Administration of medication to clients, provide education and individualized medication compliance plans to assure adherence and maintain medication monitoring per Psychiatrist instructions.• Maintenance and upkeep of medication cabinet and medication logs as per Medi-Cal requirements.• Complete client charting and documentation of activities within requirements of Department of Mental Health.• Identify problems and implement solutions related to Client community issues, promoting self-sufficiency.
