To support ex offenders and young people placed in care from challenging and vulnerable backgrounds aged 16-18.Liaising with Local Authority in the social services department and families.Liaising with other related services such as the youth offending team, social workers, police, leaving care team, doctors all in aid to support the young person.Safeguarding and protecting each young person according to their risk factors.To do shift work at a secured 24 hour unit.Supporting young people aged 16-18 in supported housing with independent living who have been reffered by social services.Monthly report writing on each young persons progress.Attending Looked After Child (LAC) review meetings with an independent reviewing officer and social worker.Registering young people in education and health services.Conduct risk assessment & Independent skills assessments.Support young people with their asylum seeking process through the home office.Support young people in youth court & any lawful proceedings.To be on call to do unannounced & late night checks on young people and to be on call to assist with any incidents that may occur at unsociable hours.
