These efforts lead to an increase in flu vaccinations by 280+ members and pneumonia vaccinations by 460 members.• Led the market through two initiatives focused on chart retrieval and education for providers and members for four STAR measures. Received a 76% response rate for DRE chart requests, an 11% conversion rate for A1C, a 26% response rate for RA and a 24% conversion rate for LIS, resulting in an overall positive impact on STAR measures. • Identified inefficiencies in existing project workflow, brainstormed areas of improvement to streamline the process, created and presented a business case to senior leadership which contributed to the hiring of an additional STARS resource.
