Provide Community Psychiatric Support Treatment (CPST) services to severely and persistently mentally ill clients. Establish therapeutic relationships, assesses complex problems, selects problem-solving interventions, and helps clients to function effectively. In conjunction with a supervisor and team psychiatrist, assess the client’s mental health and basic needs, determine the appropriate level of care and support, determine the availability of resources, and develop an Individual Recovery Plan. Implement and monitor the Individual Recovery Plan. Coordinate access to the recommended medical treatment services, vocational assistance, housing assistance, and other targeted services. Monitor the treatment progress of clients, reassess client status and the effectiveness of interventions, revise the Individual Recovery Plan, and advocate for clients. Coordinate, conduct or participate in assessments; provide crisis assistance services; delegate recovery plan tasks to appropriate service providers; and monitor overall service delivery.
