Duties, Accomplishments and Related Skills:• Provide services to youth aged 4 to 18 years and their families• Perform case management duties to include:o Drug and alcohol assessmentso Risk and trauma assessmentso Bio psychosocial assessments• Develop treatment plans and goals for the youth, family, and parents which include areas of strength and improvement, barriers to treatment and progress or decline• Help families obtain different mental health services after discharge such as group therapy and wraparound services, which include behavioral specialist, mobile therapist and therapeutic staff support • Research, collect, correlate and analyze data• Handle highly sensitive and secure documents pertaining to cliental • Work collaboratively with various organizations and school systems across multiple PA counties:o Attend Individualized Education Plan (IEP) meetingso Provide in school counseling serviceso Attend/organize tours of secondary schools for students who are found unfit for main stream classrooms• Perform crisis management which includes:o Mobile therapy if/when a child is taken to a hospitalo Develop and implement crisis plans with the family and the childo Work directly with crisis centers, hospitals, and inpatients to provide safety for the child during and after the crisis• Assist families in obtaining adequate housing, food, and medical attention when necessary• Provide advice to families regarding identifying and recognizing triggers in their child and offer de-escalation techniques• Handle paperwork to include: scheduling, case notes, treatment plans, assessments, and genograms• Maintain case notes for patient files• Participate in evaluations performed by mental health doctors• Handle psychiatric placements as well as out of home placement• Independently track my number of hours of services rendered to the client on a weekly basis
