Incorporated ways where the clients could use kitchen facilities and bring in their own food. With the elimination of the day guest services Mercy Hospice was able to really focus on being a recovery house to serve people with a history of substance abuse. This was especially poignant because the change at the height of the opioid epidemic.

Directed caregiving team in creating unified interdisciplinary care plan for each resident. Offered personalized information to families, caregivers, and residents on resident’s needs by collaborating with psychiatrists. Assisted families of residents and hosted care conferences for interdisciplinary team. Conducted quarterly evaluations on residents’ dispositions and actions. Leveraged nursing routines and recreational therapy activities to establish, apply, assess and lead community life, and afford consistent structure for residents with dementia. Assisted with feeding residents and aided nursing staff with care when residents where combative due to dementia. Creatively coordinated with maintenance team to make the dementia unit more conducive to their care. Reduced behaviors, improved care giving efforts and gave families better spaces to visit.
