Adult home health occupational therapist and physician liaison

- Assess and facilitate through direct therapeutic intervention patient's ROM, strength, coordination, functional ability and safety as related to patient's ADL/IADL performance needs.- Assess the patient's need for adaptive equipment/devices and train in their use.- Perform home safety evaluation and advise on alterations consistent with patient needs and capabilities.- Develop individualized therapeutic exercise programs to improve patient strength, functional endurance and safety within the home and community.- Coordinate care with team members including MD, RN, PT, SLP and MSW.- Educate family members and caregivers.- Proficient in the use of Kinnser documentation software

- Primary Occupational Therapist on the Acute Rehabilitation Unit working with an adult population.- Provided services as needed for the BICU, NICU, PICU, general pediatrics and outpatient rehab.- Test and evaluate patient's physical and mental abilities and analyze medical data to determine realistic rehabilitation goals.- Plan and direct therapeutic activities targeted to help individuals regain functional independence.- Advise on the use of specialized equipment to promote independence with ADLs- Perform home evaluations, assessing and providing suggestions for environmental modification to allow for better access, independence and safety.- Instruct patients and families in home programs, basic living skills and the care and use of adaptive equipment.- Work closely with the rehabilitation team participating in daily meetings, family conferences and co-treatments with physical therapist and speech therapist when indicated.- Supervise COTA in implementation of treatment plans, development of treatment strategies, interaction with patients and therapy scheduling.- Design, develop and implement staff education on the practices of Occupational Therapy within the scope of adult acute rehabilitation.
