Working in Partnership with West London CCG to set up the first holistic health check service for the community affected by the Grenfell tower fire. Responsible for: service mobilization, hiring and managing the team, partnership management and referral generation, marketing and comms and clinical and quality assurance. This role is very dynamic, high pressured and very reactive to the needs of the community and the commissioners. I have developed effective communication skills due to the variety of individuals I work with. My duties include: setting up the service, securing clinic delivery venues, , ensuring the smooth running of the day to day activities. Marketing, promotions and running social media channels. Working across the borough in a community setting, I deliver tailored one to one sessions with clients helping them to lose weight, get active and eat healthily with the aim to reduce the risk of CVD and Diabetes.This role required me to build and maintain good relationships with clients, GP surgeries and local health services which ensured suitable onward referral routes for clients. I frequently attended outreach events, meetings and presentations to increase the number of referrals into the services. I regularly used software for inputting client files, tracking progress of clients and reporting.
