CurrentMy area of expertise is in the Practice of Pharmacy relating to the dispensing error and why this error should not have occurred. This would include: *The Standard of Care incumbent on the profession*The technical and mechanical aspects of dispensing and how the violation of these standards contributed to the event to potentially include failure to adhere to related requirementsOBRA '90 required Prospective Drug Utilization Review*Technological barriers to errors *Patient Counseling Standards *Doctrines of Learned Intermediary and the associated Duty to Warn mandated of the pharmacistThis expertise extends to the pharmacy's and corporate handling of this and, particularly, previous Quality Related Events by the dispensing pharmacy team plus the involved retail location, and it's implementation - or lack thereof - of a Continuous Quality Improvement Program in response to such events, and how such lack of oversight on the part of the company could contribute to a work environment that fosters such errors. If a lack of concern evidenced by an absence of response from corporate is revealed, with subsequent lack of corrective institutional control measures imposed and maintained, then a pattern and practice of negligence, perhaps wanton, may be attached beyond the involved pharmacy team to the pharmacist-in-charge and corporate entity, all professionally and legally mandated to establish and employ such written action plans.
