• Maintain current assessment and documentation of resident health status through initial and on-going assessments. Assure up-to-date service agreements and care plans. Communicate changes in status to medical and other relevant individuals. Maintain clinical charts and progress notes using electronic health records. • Manage and oversee medication treatment and administration by ensuring medications and supplies are ordered in a timely manner, set-up, and administered according to provider orders. Assure accuracy of medication and treatment records including documentation of administration. Take appropriate action with staff regarding any problems or issues as they arise. Assist in transition from paper medication administration records to electronic.• Monitor work and performance of licensed and unlicensed staff by meeting to coordinate care and address care issues. Plan and delegate work to staff within guidelines. Interview, train and orient new employees as directed. Assist Director of Health Services with monitoring and supervision of staff as needed.

• Plan, provide, and evaluate nursing care in accordance with policies and procedures, and consult with case manager regarding changes. Assess and interpret symptoms and institute remedial measures, as well as report significant changes in condition immediately. Work with families and health care professionals to provide continuity of care. • Provide total client care including bathing and dressing, tube feeding, administration of prescribed medications and treatments via GJ tube to include tube care, tracheostomy care and suctioning, and ventilator support.

Perform all duties of Credentialing Coordinator as well as act as a back-up to direct supervisor. Perform daily and weekly reporting duties as well as delegation of work. Act as resource and subject matter expert for credentialing staff and customer service staff. Onboard and train new team members as needed. Review, assess and problem solve current policies and procedures to evaluate efficiency and effectiveness. Continually audit team members for quality assurance and adherence to accreditation standards. Process provider credentialing applications including initial mailings, review of applications and loading information into database tracking system. Request and verify missing information as necessary for credentialing purposes. Answer provider questions related to credentialing and contracting, and provide guidance regarding credentialing process. Assist in brainstorming and development of new policies and procedures.

Facilitate Prior Authorization requests with Doctor office staff and Insurance companies. Assist in obtaing records and relevant clinical information necessary for review and approval of Prior Authorizations. Assist patients in placing medication renewal orders, understanding insurance benefits and requirements, and explaining medication and copay information. Trouble shoot insurance rejections and billing problems to obtain paid claims. Assist other staff members with questions related to insurance procedures and requirements. Pick, pack and ship medication orders. Check in and maintain inventory.

Review reqests for Prior Authorization based on set criteria determined by plan. Assist health care providers and patients in determining necessary steps for coverage of medication. Explain plan provisions.
