The HSP maintains contact with mental/behavioral health clinicians- therapists, psychiatrists, psychologists, nurses, and other professionals regarding each client's needs, behaviors and response to treatment, assuring that all services are scheduled and delivered in a timely manner and that clinicians are made aware of behavioral progress or lack thereof. I work to improve the viability of the youth focused needs and improve the quality of life for residents. This is accomplished by increasing residents’ access to services and by facilitating their participation in programs that enhance their physical, social, and mental well-being.As a Service Coordinator, I work to provide the day-to-day servicecoordination for families and children in the program. Duties include reviewing clinical reports on the children referred, meeting with family members and consumer to assess the strengths and needs of the family and child, and together developing a plan for service. I collaborate with existing behavioral health providers, and other community and local service resources, traditional and non-traditional, to assist the family in implementing the service plan.
