Developed and Implemented Complex Geriatric Psychiatric Care with Dementia Education and Care Plans for Senior Patients within the Acute Inpatient Psych Unit.Reviewed high users for psychiatric services within Chinook Regional Emergency and Inpatient Unit. Developed with community managers a team that included all members of the community to develop safe care plans to support the Complex patient in the community but to also improve communication between partners and the hospital.Working with Community Partners to develop and implement a seamless delivery (soft hand-off) of services post discharge.Implementation of Discharge/Flow Rounds, Leadership RoundsCompleted multiple quality improvement projects including:Mental Health Environment Inpatient Care Checklist to increase safety and reduce risk to patients and staffCompleted site practice review in conjunction with Infection Prevention and ControlRe-introduced and implemented Hand Hygiene and auditsInitiated Safe medication practices involving Methadone, narcotics, and benzodiazapinesReduced patient harm by focusing on falls, medication process review, discharges in a timely manner.Followed up with all RLS and Mysafetynet reportsManaged major renovation project of half the unit while still operating within that unit.Completed a Quality improvement review to reduce risk.
