Provide intake and assessment services to veterans to determine eligibility for assistance program. Assist veterans with obtaining homes and needed resources to maintain housing. Schedule veterans for assistance programs and needed services. Maintain daily logs. Collect and maintain data based statistical information (HMIS) and input data information into software programs. Develop and maintain housing plans and budgets with veterans. Attend bi-annual Regional VA Conferences and monthly VA calls.

Collect and maintain updated client information in the Electronic Medical Records systems. Collaborate with physicians, therapist, other staff, and community resources to ensure that client needs are being met.

Investigate allegations of child and/or adult abuse, neglect, and exploitation. Coordinate placement of children into foster homes when removal was court ordered. Facilitate placement of juveniles in state custody to appropriate levels of care. Provide expert testimony in court proceedings. Monitor conditions set by court systems. Conduct a social assessment of the client by interviewing the client in the clinic and/or home setting to determine services and resources needed, prepare treatment plan based on the social assessment in conjunction with nursing personnel and other care givers of the agency or community, provide indicated services or assist patient in acquiring services within the community.

Discharge Planner-Work with the patient and treatment team to coordinate patients next level of care post hospital discharge. Assess, plan, implement and evaluate the needs of patients for discharge planning activities. Maintain updated patient data within the Electronic Medical Records. Work collaboratively with patients, families, hospital staff, physicians, and community resources to ensure that the patients discharge needs will be met.Dialysis Tech-Obtained patients vital statistics and recorded accordingly. Perform venipuncture to obtain access for hemodialysis treatments. Monitored Hemodialysis machine and provided direct patient care during treatments.
