Home Advantage Collaborative (HAC) Program Coordinator July 2017 - March 2019● Coordinated logistics of a subsidized housing program for 37 families a year, including management of $809,000 of grant funds, ultimately eliminating a $147,000 surplus and assisting more families.● Led and provided logistical supervision to 5 cross-functional team members, ensuring placement of 13 families to stable, permanent housing after 12 months.● Provided case management to 16 families and collaborated with them to establish a path out of homelessness and ensure their follow through and consistent progress.● Partnered with IT team to establish program-level data collection, analysis and repeatable program metrics. Building frameworks in spreadsheets that improve case management team’s process.● Collaborated with Housing Director on writing grant renewal applications, generating reports and supporting information, resulting in two annual funding renewals (total of $1.3 Million).● Partnered with external agencies to build improved referral pathways, creating a streamlined experience for families experiencing homelessness to enroll in the program.● Monitored for gaps in client care to drive client-centered program changes.Home Advantage Collaborative (HAC) Case Manager March 2016 - July 2017● Provided weekly clinical case management services to 20 families, working with clients on development of a long-term housing plan and transition into permanent housing.● Conducted initial assessments with clients at entrance of the program to establish Individual Housing Service Plan (IHSP) based on their level of need and intervention, ensuring client accountability.● Consulted with HAC Employment and Housing Specialists, to reduce barriers to clients' stable housing.● Advocated on behalf of clients to maximize access to mainstream benefits, influencing 95% of families to increase their assets and income necessary for long-term housing success.
