Develop and administer custom care management plan and services for client / patients going through end stage of disease /or terminal illnesses and living with a mental health diagnosis. Family members maintain that quality of care improved the quality of life for client / patients immeasurably during a client / patient's final months.Able to manage any and all logistics and requirements for a client / patient's medical Chemo / Radiation Therapies including: medical, psychiatric, and occupational therapy appointments; medical documentation and reporting; labs and testing, medication management nutrition; and alternative homeopathic therapy research.Develop and maintain online reporting to family to ensure timely delivery of critical data and information pertaining to a client / patient's diagnosis, medical options presented, current condition, changes to condition, needs expressed by client / patient, and detailed responses to inquiries made by family.Advocate for client / patient / consumer by negotiating "mutually agreed upon" client / patient care plan with family members to ensure client / patient's (final) wishes are honored, while simultaneously addressing and integrating measures to satisfy the family's concerns. Components addressed include non clinical case management, oversight and critical care, client /patient/consumer comfort, and the family's need to maintain continuous communication flow regarding client / patient / consumer status.Committed to initiating family awareness campaign and lead discussions to develop most effective care plan to address behavioral healthcare needs of clients of all ages, including frail elders, who desire to remain living at home with community resources.Convene multi-generation family interventions to engage and motivate family and close family friends to act and attend to the immediate critical healthcare needs and legal affairs of clients that require additional support and at risk family members.
