    •Access patient charts via EMR/Epic- Access and navigate the electronic medical record to identify pertinent information.•Complete referrals to determine if patients meet criteria for potential admission- When a patient is ready for discharge, or is a difficult placement, a social worker or discharge planner contacts a Clinical Liaison to have them look at a patient remotely to see if said patient meets criteria for admission. This may allow patients to conserve insurance days, free up ICU beds at facilities or allow families extra time to make decisions. Often, the ltach is a steppingstone to allow more time to become medically stable prior to discharge to a rehab facility, skilled nursing home or home. Once the referral is received, I access their chart and evaluate their entire length of stay to understand their hospital course, where they started, and where they are now. Next, I enter all pertinent information into our system to verify the patient meets criteria for admission. •Complete conversion notes and full updates in compliance with job requirements- every day the patient is assessed through the electronic medical record (EMR) to ensure the patient continues to meet criteria for admission. Every other day as well as day of planned admission a full assessment is completed to include updated imaging, labs, vitals, medication lists, procedures etc. Every other day, an updated note of the last 24 hours is completed. This documentation is critical to ensure only patients that meet criteria are admitted to our facility to maximize reimbursement to meet the business plan.

    •Establish mobility programs- Early mobilization of critically ill patients is important in preventing further complications of their current condition. As part of the mobility program therapists evaluate each patient to determine their ability to move around their environment. Early mobility mitigates complications such as acute DVT (blood clot), muscular weakness, wounds, and complications of mechanical ventilation such as Pneumonia and Delirium which results in decreased ventilator days, and decreased hospital length of stay.•Educate on and assist with establishing splint programs along with the appropriate splinting devices- Upon admission each patient is evaluated by physical and occupational therapy and is monitored/treated throughout their stay. When a patient is critically ill many side effects occur. As therapists it is our duty to maintain range of motion to all joints to allow proper hygiene to occur, prevent joint contractures and prevent wounds. I work with the interdisciplinary team to look at the whole picture and assist with choosing proper equipment to best care for the patient. Equipment includes splinting, bracing, bed choice, cushion selection, adaptive equipment such as walkers, wheelchairs. Through this process it is imperative to work closely with outside vendors and physicians to ensure insurance coverage. •Provide caring treatment for chronically, critically ill, and ventilator-dependent patients- When a patient is admitted to our facility, I not only treat the patient, but the family. An LTACH level of care is something most families will never experience, but for those that do, it can be a very scary experience. Working with patients and meeting them at their current level is crucial. Along with patient treatment educating the family and the patient is the most important aspect of their recovery. Providing appropriate education to all parties facilitates trust in providers, empathy for the patient and trust in the process.

    • Provided treatment for various musculoskeletal and neurological conditions using various techniques- Throughout my career as a therapist I have had the opportunity to treat numerous diagnoses including total joint replacements, sports injuries, stroke (CVA), Guillain-Barre, vertigo, surgical repair of injuries, and balance disorders. Treatment of these diagnoses utilize a variety of techniques including but not limited to modalities such as electrical stimulation, mechanical traction, ultrasound, Sound assisted soft tissue mobilization (SASTM), heat therapy, cold therapy, taping strategies, therapeutic activities, therapeutic exercise, neurological re-education and manual therapy techniques. • Work conditioning programs and Aquatic Programs- During my time as an outpatient therapist I had the opportunity to provide services for work conditioning as well as aquatic therapy. Work conditioning is a treatment in which the clinician creates a program for the patient to facilitate successful transition for return to work. The patient would come to the clnic and perform tasks similar to their job duties such as working overhead on a ladder, pushing/pulling larger loads, standing for prolonged periods of time, etc in a controlled environment with progressive increase in frequency and duration to simulate the work environment. Aquatic therapy is a form of physical therapy for patients that may have weight bearing restrictions or be extremely pain sensitive. In aquatic therapy buoyancy is utilized to relieve weight and allow more movement with less pain. • Assisted with marketing of new and existing clinics to the public and physicians- As new clinic locations would open; I worked with our marketing team and clinic managers to provide lunch and learn meetings with physicians. We worked closely with running groups to ensure proper footwear, offer injury prevention programs, and free stretching clinics after races.
