As a Transitional Care Manager at Complete Health, I provide care for geriatric patients discharging from an acute care setting, ensuring a smooth and safe transition to their homes or other facilities. With the help of my team, I coordinate services, resources, and education for patients and their families, as well as monitor and evaluate their outcomes and satisfaction.I have six years of experience in geriatric care, including two and a half years as a Registered Nurse and a Certified MDS Coordinator at Diversicare Healthcare Services Inc. There, I was responsible for the rehab floor nursing for post acute care, wound care, and the completion and submission of the MDS assessments. I also participated in quality improvement initiatives and audits. I graduated with an Associate of Science degree in Registered Nursing from Central Alabama Community College in 2019, after completing my Licensed Practical Nurse training in 2018. Nursing is my passion, and I have a deep appreciation and love for the care I provide. I am always eager to learn new skills and knowledge, and to contribute to the well-being and dignity of my patients.
