Medical Social Worker
Current* Completes initial, 90-day and annual comprehensive psychosocial assessments on all patients following regulatory and professional guidelines* Collaborates as a member of the health care team to develop, implement and evaluate on-going patient care plans* Facilitates new patients and their families admission and orientation to dialysis care addressing both patients and their family member needs in the adjustment to the dialysis regimen* Makes referrals for resources in the community advocating for patient access to services such as vocational rehabilitation, in-home care, housing, hospice and transportation as needed* Provides supportive brief counseling interventions including crisis interventions for stresses stemming from dialysis and chronic disease and its impact* Coordinates with kidney transplant outreach representatives from UC Davis, Sutter Health/California Pacific Medical Center, & University of California San Francisco in managing patients’ transplant referral and patients’ transplant status on the kidney transplant list* Collaborates closely with other Satellite Dialysis Central Valley Social Workers in providing unit coverage, case consultation and referrals to county resources in San Joaquin and Stanislaus Counties
