Responsible for coordinating community benefit and relations initiatives and activities. Coordination and implementation of community-based initiatives to improve the health status of residents within the community, to develop cost-effective solutions, and build a positive image for SelectHealth and its partners in the community. Provide detailed coordination of activities in the community by providing support during actual initiative activities to enhance the community experience, provides direction and leadership for process and implementation with internal and external entities to include, but not limited to, not-for-profit agencies, government, and business groups. Works independently on complex projects within the SelectHealth team. Promotes SelectHealth as a not for profit organization.

Investigate current public health concerns, specifically diabetes and heart disease, to determine best practices and continuity of care across Salt Lake County.

• Facilitate general evaluations and test on patients to determine the best quality therapy and treatment needs.• Collaborate with Exercise Physiologist, Surgeon, and Cardiologist to create the overall treatment• Complete documentation, clean and inspect equipment• Evaluation and treatment of patients post open heart surgery or MI/Stent procedures• Teach pre-operative guidelines and expectations to patients• Evaluate vital signs, heart rhythms, and signs and symptoms of any complications• Teach discharge instructions clearly and succinctly

- Responsible for coordinating all aspects of the Hospice Volunteer Program in the Provo office.- Recruiting, training, supervising, and retaining volunteers.- Placement of volunteers in an efficient and timely manner to meet patient/ family/ hospice program needs.
