Youth In Transition Worker
Current• Builds rapport with youth ages 16-24 who are leaving care of a Children's Aid Society to form long-term case management and supportive counselling relationships• Provides advocacy and service connection based on the goals of each youth and an assessment of their needs in a path towards independence• Assists youth for service coordination and goal achievement in: housing, employment, education, mental health, health, life skills, legal, supportive counselling, etc.• Works with youth one-on-one to complete tasks, attend meetings and develop necessary skills, such as budgeting and money management, meal planning/grocery shopping, job search, research skills, housing search, organization and routine building, etc. • Conducts case management, completes accurate case notes, maintains files accordingly, and gathers statistics for ministry reporting• Provides outreach, presentations and allocates time for community committee membership
