Serve as a liaison between the patients/families and primary care providers office in scheduling appointments when parents are unable to call. Ability to navigate Epic, electronic medical records, to locate child’s immunization records, write progress notes on each patient interaction, and upload patient files received from outside sources. Educate parents/guardians on the importance of vaccines & well child examinations to prevent certain diseases and maintain child’s overall health. Ability to communicate with parents/guardians to identify barriers that might be inhibiting preventative health care; transportation, food, housing.
