As a Level II Fieldwork student at Kennedy Health Care Center in Washington Township, NJ in the sub-acute rehabilitation center, I became gradually responsible for the entire OT process from start to finish with guidance from my supervisor. Most common diagnoses on my caseload included hip/knee arthroplasties, COPD, PNA, CVA, cardiac conditions, cellulitis, and CKD. I completed full evaluations on patients with assessments such as occupational interview, MMT, ROM screening, modified FIM scale, observation, sensory assessment, vision screening, etc. and completed evaluation write-ups including goal setting and discharge planning. I implemented functional interventions with my patients including lower body ADLs, upper body ADLs, functional mobility, transfer training, edema management, energy conservation techniques, etc and completed daily documentation reporting progress. I collaborated daily with the interdisciplinary team and also attended weekly care conferences with patients, family members, nursing, PT, SLP, and LCSW. In addition, I completed discharge planning and wrote discharge summaries for each patient on my caseload.

As a Level II Fieldwork student at Children’s Specialized Hospital, Outpatient Clinic in Toms River, NJ I was strategically provided with more and more responsibilities throughout my 12 week clinical. I began with observation and finished by treating a full caseload independently, with assistance from my supervisor when needed. The children on my caseload ranged throughout a variety of ages and diagnoses. Diagnoses of the children included autism, sensory processing disorder, ADHD, CP, feeding difficulties, fine motor incoordination, gross motor incoordination, behavioral disorders, etc. I completed comprehensive evaluations with each client including parent medical history interview, general client observation, clinical sensory observation, and standardized assessments such as Sensory Profile, Sensory Processing Measure, BOT-2, and PDMS-2. I wrote up evaluation reports including client strengths, difficulties, assessment results, and goals. I was responsible for developing detailed daily treatment plans for each child on my caseload and implementing them in the setting using clinical and critical reasoning and writing daily documentation after each session to report progress. I also completed discharge planning and wrote discharge summaries for each client. In addition, throughout my clinical fieldwork I worked daily with parents of clients in the therapy gym to promote carry over and foster the most progress for their child.

As a Preschool Special Education Teacher at Little Vikings Learning Center in Lewes, DE I was responsible for teaching students ages 3-5 with disabilities in an inclusion classroom. The diagnoses of my students included developmental delay, preschool speech delay, and autism. I developed daily lesson plans for preschool aged students with and without disabilities in all subjects including reading, math, music and movement, science, physical health and development, social skills, self-help/toileting skills, and Handwriting Without Tears. I also evaluated student’s academic, behavior, and developmental performance in order to develop student IEPs. I conducted IEP meetings with an interdisciplinary team including families, PT, SLP, social work, administration, and nursing and implemented IEPs daily in the classroom. In addition, I was chosen as the Professional Learning Community Leader of my preschool teacher team and attended monthly professional development trainings.
