- Providing case management services to assist youth in DCF/state custody from the Newport District as they transition to adulthood and acquire the skills to be self-sufficient adults in a community setting.- Assessing and assisting youth in identifying their goals; developing a strengths-based plan of care to help them meet their goals; and identifying life-long connections and support. - Assisting clients in developing individualized comprehensive plans for making the… Show more - Providing case management services to assist youth in DCF/state custody from the Newport District as they transition to adulthood and acquire the skills to be self-sufficient adults in a community setting.- Assessing and assisting youth in identifying their goals; developing a strengths-based plan of care to help them meet their goals; and identifying life-long connections and support. - Assisting clients in developing individualized comprehensive plans for making the transition from state custody.- Outreach to DCF and youth in care, collaborating with existing custodial guardians, attending DCF case plan reviews, and consulting and contributing to the development of the DCF case plan. Case management efforts include, but are not limited to: • Assist youth in identifying their goals;• Conduct formal assessment of the youth’s preparation for adulthood;• Assist youth in developing a plan that uses their strengths to help them meet their goals;• Assist youth in attaining sustainable employment;• Assist youth in identifying their goals to complete secondary education and prepare, enroll, and complete post-secondary education;• Assist youth in accessing and maintaining affordable housing;• Assist youth in accessing health care;• Assist youth in identifying and developing lifelong connections and support from caring adults.• Creating and support of opportunities for youth to participate in community service and leadership activities, including the state Youth Development Committee. • Providing access to the supports and services called for in Act 74, the Youth in Transition Law and its associated rules.
