Care Management Supervisor
CurrentProvide daily oversight and supervision of care management clinical and non-clinical staff to ensure that members receive comprehensive, cost effective, high quality, coordinated health care services while achieving program outcomes and process metrics. Responsible for the delivery of care management services that include transitional care, disease management and utilization management for members.Establish and sustain collaborative relationships with participating providers and their practice staff to facilitate CCNC (Community Care of North Carolina) staff activities to develop and implement quality improvement, care management, and population health initiatives. Establish annual work plans at the team and individual level with goals, objectives and metrics geared towards meeting company objectives. Assure quality, cost and performance objectives including plan specific Service Level Agreements are achieved.
