• Initially provided full-time social work coverage for the surgical intensive care unit and one inpatient medical team. Also provided coverage for the entire surgery service on an as-needed basis. Later transferred to primary care and provided full-time coverage for two outpatient medical clinics, comprising a total patient caseload of eight attending physicians.• While working in the inpatient units conducted psychosocial assessments and high-risk screenings in order to coordinate detailed, patient-focused discharge planning with referrals to multiple facilities and vendors in order to arrange for nursing home, long-term acute care and short-term rehabilitation placements; home health care, hospice, home IV antibiotics, and outpatient hemodialysis arrangements; and referrals for homemaker services, meals on wheels, chemical dependency treatment, medication assistance, transportation, and durable medical equipment.• While working in the outpatient clinics conducted psychosocial assessments, high-risk screenings, crisis intervention, ongoing case management, and solution focused brief therapy. Assisted patients and their families with identification and referrals for specialized services such as chemical dependency treatment, financial and employment counseling, legal services, and housing support. • Was recruited to the Ethics Committee to assist with ethical concerns and dilemmas related to direct patient care, medical treatment and planning, end of life issues, and patient/family conflict, as well as ethical concerns regarding ethics topics in health care and interpreting policy in relation to health care and medical research.• Was selected to participate in two Lean Process Improvement Workshops in order to assist in identifying methods to improve patient safety, quality of care, and efficiency during the inpatient stay and discharge planning processes. Co-authored a Resident Physician Handbook to be used hospital-wide.

• Provided full-time social work coverage for the intensive care unit and telemetry unit. Also provided social work coverage for the oncology unit and emergency room on an as-needed basis.• Conducted clinical utilization reviews for the intensive care unit, telemetry unit, and oncology unit in order to assess the validity of both patients’ admissions to the hospital and continued stays within the hospital using McKesson Interqual® criteria and guidelines. Contacted EQ Health Solutions for Public Aid cases and multiple other private insurance HMOs and PPOs in order to provide them with detailed clinical reviews in order to certify both hospital admissions and continued stay.• Was an instrumental component in weekly multidisciplinary discharge rounds in order to closely monitor and provide for existing and potential patient needs and to assist in decreasing overall length-of-stay in accordance with DRG regulations.• Attended monthly Case Management Committee meetings and presented extended-stay clinical reviews to the Chief Medical Officer and Chairman of the Case Management Committee in order to identify potential quality issues and delays in service for Medicare admissions exceeding the DRG length-of-stay by ten days or more.• Was appointed the McKesson Interqual® super-user for the Case Management Department.• Other social work duties performed are commensurate with those listed for inpatient units for Jesse Brown VA Medical Center.

Also provided coverage for the emergency room, general surgical unit, acute rehabilitation unit, and mother-baby/labor and delivery unit on an as-needed basis.• Continually advocated for patients’ rights in their health care planning and decision-making and strived to provide patients with necessary education and tools related to their diagnoses to assist them in making informed decisions regarding their health care needs.• Chaired the Social Work Education Committee and fostered relationships with outside health care agencies to obtain updated educational information and Continuing Education Units.• Other social work duties performed are commensurate with those listed for the inpatient units at Jesse Brown VA Medical Center and Thorek Memorial Hospital.
