In this role, I guide a growing team of nearly 100 clinical and non-licensed professionals responsible for providing value-based care for dialysis and CKD patients across the US. I manage service delivery and strategy; administer our operating budget; lead instructional design and patient education programs; and build critical partnerships and relationships in our market. Key Achievements: • Played a key role in a collaborative effort to secure our NCQA accreditation, engaging with team members and stakeholders across the enterprise to ensure the process was successful. • Championed a home first philosophy, focusing on removing barriers to care and driving healthcare equity. • Engaged with and developed employees from a remote workspace, assessing the unique needs of each program to support growth and ensure resources were allocated appropriately. • Partnered with Accenture on the development and launch of CareTeamHub (a Salesforce product). • Grew our Renal Care Coordination program from 5 to more than 40 across individual practices, as well as designed and distributed onboarding resources and program supports for practice partners.

During this time, I engaged with the medical office and our Clinical Interventions Lab to identify process improvements, service gaps, and program enhancements. I led clinical design and implementation of pilot programs. I worked closely with our actuary team as a clinical data analyst to look for key clinical insights leading to areas of opportunity to improve patient care and outcomes. Key Achievements:• Served as a Subject Matter Expert (SME) on CKD-specific chronic disease management programs marketed to large payors throughout the US. • Introduced SOPs to create a consistent operating model and expectations for programs.• I partnered with Actuary team to provide clinical data analysis and key insights leading to opportunity for improving patient care while reducing medical spend.

Eastern United States

As Program Manager, I led operations and strategy for nine Renal Care Coordination programs along the East Coast. This included coaching and developing RNs embedded in practices around the region, identifying gaps and concerns specific to their market and practices, and monitoring clinical practices and patient outcomes for CKD Stage 4 and 5 patients. Key Achievements: • Consulted with practice physicians to review metrics and trends, identifying strategies for improving home penetration rates, permanent access rates, and 120-day survival rates. • Demonstrated the ROI of our program by creating a comprehensive analytical review. • Partnered with the Senior Director on process improvement projects and program enhancements.

When I started with the organization, I was in charge of developing strategies and operating plans to ensure positive outcomes for dialysis and chronic kidney disease (CKD) patients. I collaborated with our executive team on defining our program vision, introducing best practices, and monitoring clinical quality. I also built relationships within the local community and facilitated patient education seminars to assist in understanding chronic disease. Key Achievements: • Transformed care for patients by introducing a pilot concept that eventually led to a value-based care model, serving as the fifth Renal Care Coordinator in the US embedded within a practice. • Delivered a 400% improvement in home care options in my first year as an advocate for change and the liaison to dialysis facilities and transplant centers in the area. • Established a peer mentoring model to support patients in managing chronic disease. • Doubled vascular access rates by building relationships with providers to identify areas of improvement, as well as educating nephrologists about surgical timelines to encourage earlier referrals.
