Years of healthcare experience, to include population health management, value-based care, and direct patient experience that provided individual and community health education, care coordination, patient navigation and self-management support related to chronic conditions, nutrition, healthy behaviors, emergency room use, and resolving barriers such as, SDOH-related barriers. My background includes inpatient and outpatient settings, skilled nursing facilities, behavioral healthcare settings, federally qualified health centers, and provider offices. I have led teams in Medicaid, Social Security Disability Insurance (SSDI) and Supplemental Security Income (SSI) eligibility, as well as Qualified Health Plan (QHP) enrollment. More recently, I was a Clinical Program Director who partnered cross-functionally to support key stakeholders in meeting clinical outcomes by driving program and platform implementation, leading clinical workflow design, supporting population health platform clinical informatics, and developing the expertise of all patient-facing teams (clinical and non-clinical) through training and education. I have a passion for creating equitable and inclusive programs, both for patients and staff, through empathetic and action-driven leadership across diverse departments and cross-functional teams.
