Director, Peerlinks
CurrentResponsible for the growth, sustainability, and accountability of PeerLINKS, an innovative program that strives to reduce readmission rates to psychiatric hospitals by providing enhanced peer support services (www.namisandiego.org/services/peer-links/). Essential Duties and Responsibilities:- Ensured successful start up of program by defining scope and vision of program, building a talented and motivated interdisciplinary team, developing policies and procedures, working collaboratively with the funding agency (County) and evaluations team to finalize metrics, as well building relationships with the various hospitals and crisis homes that refer participants to our program.- Monitor and lead day to day operations of the PeerLINKS program.- Identify, hire, and oversee training of all new employees.- Supervise and motivate a diverse team of a dozen full-time employees to provide effective and high quality services.- Provide presentations and outreach to community members, providers, and Behavioral Health Advisory Board, to increase awareness of program and its positive outcomes.- Analysis of data reveals that over the past two years, participants had statistically significant improvements from intake to follow up in a variety of outcome measures, including: level of recovery, mental health, physical health, social health, resilience, depression, anxiety, memory/cognition, substance use, and active support network.- Preliminary data finds evidence of reduced readmission rates to psychiatric hospitals; specifically, participants showed a 30% reduction in readmission rates (30-day recurrence rate).
