I work with children that are placed in Pathway foster homes. I help with youth's adjustment into foster care and placement in the foster home. I assist with transportation as needed. I meet with the youth weekly, bi weekly, or once a month depending on their level at placement. Many of the youth that I work with have some type of mental health diagnosis. I keep track of my visits with the youth. I develop service plans for the youth which keeps track of the services that are provided to the youth. Monthly reports are sent to some agencies to monitor the youth's progress in the home.

I worked with abused and neglected children that had been removed from the custody of their parents. I visited once a month with the youth and foster family to monitor progress in placement. I developed case plans which listed the services that the family and youth needed to complete before the youth could be returned back into their custody. Basically, I provided the role that the bio parent should fill. I maintained contact with the school to monitor progress in school. I worked with the foster parents to make sure youth's medical, physical and emotional needs were being met. I attended 6 month court reviews to share progress with Family court as to progress youth made in foster home, school and community. I also discussed progress that bio family had made.
