● Played a pivotal role in the successful launch of a new community health center, spearheading program development, strategic planning, operation and management. The CHC provides primary care for attached patients and programming tailored to bolster community well-being, particularly focusing on mental health, substance use, and folks aged 65 and above. ● Coordinator for population health programming. Collaborating with interdisciplinary clinical and allied health team to develop, facilitate and evaluate health promotion activities including: health screening, skills building and education to improve patient and population health outcomes. Content was of diversity, inclusion and equity and Indigenous determinants of health aiming to inform and guide decision-making processes.● Developed a detailed project proposal and implementation plan, including conducting an extensive environmental scan and curating relevant resources.● Collaborated closely with knowledge translation experts to ensure the primers effectively communicated essential information to the target audience.● Partnered with a health promotion analyst to engage HEIA users in consultations, evaluating current utilization, identifying barriers, and gathering feedback to inform future enhancements of the program.
