As a Triage RN I triage calls from patients and family members and provide advice or dispatch the on call nurse for urgent needs. Work collaboratively with multidisciplinary team, including Medical Doctors, Administrators, Social Workers, Registered Nurses, LVN’s, and Chaplains to provide the most comprehensive and patient specific services to Hospice care patients and their families. I schedule visits, order and discontinue medications based on physician orders, coordinate equipment deliveries and pickups, provide support to family members, direct lvn's based on their assessments, and document all calls in a timely manner.

Plan, Coordinate, Provide, and Evaluate care for a group of assigned patients.Administer medications, complete treatments, and assess patients. Communicate with physicians and families. Direct CNA's and other ancillary staff for the residents well being as necessary.
