Community Support Worker
My previous duties were:- talking and responding to Service Users phone calls, face to face, dealing with home care referrals via the contact centre, starting the initial assessment process by liaising with members of staff, doctors, nurses, Service Users, their family members, day centre staff, potential care homes, word processing various correspondence i.e. letters and inputting relevant data onto the assessments, record my findings onto case notes and review care packages face to face with service users and their families. I have been fortunate that can use wide range of my experience in different positions making foundation in all types of administration, management, translations and customer services. My interpersonal skills and communications with others are first class and I can deal with most people and any situation, whether planned or thinking on my feet. I have experience in completing Combined Community Care & Support Plan and carers assessments and am aware of the legislation that underpins both types of assessment, for example, Care Act 2014. Having a background in administration and a good English education from school I am quick at adding information gathered to Framework and commissioning any appropriate service. I have experience of commissioning services for older people and their carers, which includes home care, residential and nursing care placements and direct payments. As a Community Support Worker I regularly commission, review and close cases with the exception of those that I care manage for a variety of reasons like regular respite or complex direct payment.
