The HCT provides outreach services to homeless Veterans. The Homeless Team will help Veterans link with medical, psychiatric, substance abuse and housing resources as needed. The Homeless Care Team builds on each Veteran's strengths to prevent homelessness or address the issues that led to homelessness. The Homeless Care Team (HCT) works closely with other VA and community resources to provide medical, psychiatric, and substance abuse treatment as well as housing.
