Providing Medication Assisted Treatment for Alcohol Use Disorder and Opioid Use Disorder. Empowering patients with substance use disorders through Motivational Interviewing to elicit behavior changeSuboxone Waiver

In this role, I provided primary health care to patients of the skilled/transitional/long term care areas at this division of HCR/Manor that is comprised of nurse practitioners placed in various nursing facilities. I worked closely with nursing staff, physicians, and specialty providers on patient condition management, and I was responsible for admission histories, physicals, assessments, diagnostic test requests, test interpretation, plan of care development, and patient counseling in a facility with 102 beds. In addition, I performed physical exams, operated medical equipment, updated and designed treatment plans, monitored patients, supported 4 physicians, and maintained documentation.Selected Accomplishments:• Met or exceeded all metrics for time management, document preparation, quality of care, and workflow• Contributed to the decrease in rehospitalization rates from more than 38% to less than 15% by taking on the r role of senior or lone nurse practitioner at facilities and improving early identification of health issues.• Reduced acquired pressure ulcers/skin issues by implementing earlier intervention for wound care.• Exceeded required patient visits to increase the quality of patient care.• Enhanced rehabilitation potential of patients, resulting in successful discharges to patient home.

I provided urology care for skilled and long-term nursing patients for this division of American Kidney Stone Management that provided care to urology patients in nursing facilities. I was actively involved in the development of practice procedures for this new program.Selected Accomplishments:• Improved patient satisfaction and quality of care by reducing the need to transport patients to urologists.

During this time, I carried out primary care for patients in 3 separate skilled/transitional and long-term nursing facilities, I averaged 12 patients per day, working with three physicians, and I was responsible for admission histories and physicals, patient assessments, diagnostic testing, and care planning.Selected Accomplishments:• Met or exceeded all goals and served as a resource person for nursing and healthcare professionals.• Decreased rehospitalization rates to less than 10% through early identification of health issues.

I was promoted from Charge Nurse to Nursing Supervisor on the Skilled Care Floor of this transitional and long-term care nursing facilities with 98 beds. My role involved assessing patients, performing care planning, and developing discharge goals for patients. I supervised 15 nursing personnel, oversaw building safety, and resolved personnel issues, and I led all staff training, monitored staff performance, prepared daily morning patient reports.Selected Accomplishments:• Played a key role in the facility earning a perfect score on the Ohio Board of Health State Survey for Nursing.• Consistent met or exceeded all performance objectives and earned superior evaluations.
