The focus of my PhD research, conducted at Cranfield University, was an evaluation of the application of Hazard Analysis methods to Patient Safety. While working at the National Patient Safety Agency (NPSA). I helped produce research based interventions, including leading the development of guidance on safer ambulatory syringe drivers.
